Founded by survivors of Liberia’s civil war, Last Mile Health (LMH) (known in Liberia as Tiyatien Health) is committed to saving lives in the world’s most remote villages. Working in remote jungle communities cut off from even basic life-saving health services, LMH is building a health system that reaches everyone—by bringing health care to villagers’ doorsteps. LMH trains community members to be health practitioners for their villages and connects them with rural health clinics to offer every person access to the care they deserve. These Frontline Health Workers are able to prevent, diagnose, and treat the top ten most life-threatening health conditions. LMH plans to leverage this experience to help the Liberian government design, deploy and sustain a national network of community health workers that can have a transformative impact on maternal and child health outcomes across Liberia.

Over the past six months, an unprecedented Ebola outbreak has demonstrated that weak, inequitable health systems leave everyone vulnerable. To match a historic crisis with a historic response, LMH is launching a significant expansion of the organization’s rural healthcare model, integrated with a package of Ebola treatment and health facility strengthening efforts across the southeast. This project aims to deploy a comprehensive health system—from life-saving essential primary care services in remote communities to inpatient Ebola treatment and tertiary care in health facilities—and to work with the Liberian government to sustain that system and expand it to the rest of the country in the coming years.

GENERAL POSITION SUMMARY:

LMH is building a health systems policy and planning team, based in Liberia’s Ministry of Health (MOH) in Monrovia. This team will provide technical support to the MOH to marshal a comprehensive Ebola outbreak and transition response that strengthens the overall health system in the long-term for everyone’s benefit – no matter where they live. LMH will help design and implement a nation-wide Community Health Worker program and ensure national health system improvements are realized at the community level. To achieve this, a health systems policy and planning team consisting of highly-motivated, diplomatic, and experienced specialists will provide technical advice to MOH officials, support the implementation of community health programs in coordination with other NGO and institutional partners, conduct focused capacity-building projects, and provide long-term analysis and thought leadership for the Liberian public health system.

The Director of the Health Systems Policy and Planning team will be a trusted advisor and the principal institutional liaison to senior MOHSW officials, as well as provide technical leadership and management to the team itself. The Director will help the MOHSW direct attention and resources towards national community health priorities outlined in the national Community Health Roadmap such as national deployment of community health workers. The Director will initiate and steer new projects, leverage the appropriate expertise within the Health Systems Policy and Planning team to ensure these priorities are featured in the government’s decisions ranging from health financing, human resources for health, and clinical protocols. The Director is also responsible for overseeing all aspects of program management activities, including but not limited to program planning, budgeting, staffing, qualitative and quantitative analysis and reporting.

ESSENTIAL JOB FUNCTIONS:

Provide Strategic Support to MOHSW

  • Serve as a trusted advisor to MOH to ensure that major new and ongoing Ebola response efforts are designed and implemented to strengthen all levels of the public health system over the long-term.
  • Work alongside senior MOH officials to identify community health system strengthening initiatives and leverage the expertise of the appropriate health systems policy and planning specialists to support their implementation. This includes identifying technical assistance needs within MOH and with relevant partners such as the Clinton Health Access Initiative and the World Bank.
  • Actively participate in planning and coordination meetings among senior MOH officials and other stakeholders to identify and mobilize resources for community health priorities such as the expansion of a national community health worker program and other components of the national Community Health Roadmap.

Program Management

  • Along with the organization’s senior leadership, assist in setting the overall direction and goals of the Health Systems Policy and Planning team, defining targets, setting aggressive milestones and measuring program performance.
  • Manage the operational and administrative aspects of the Health Systems Policy and Planning team, including budgeting, human resources and internal and external reporting requirements.
  • Prepare team members’ workplans with clear objectives and achievement benchmarks, long-term and short-term priorities, implementation plans, financial projections and tools for evaluation.
  • Ensure work streams effectively address health system weaknesses at the community level and contribute to Liberia’s broader MDG progress and other health sector goals.

The Community Health Specialist is an experienced and thoughtful public health expert with an intimate understanding of health system challenges at the community level, as well as the benefits and limits of greater decentralization of care. This individual will play a pivotal role in partnering with senior MOHSW decision-makers and key stakeholders on the effective implementation of community health priorities, particularly the opportunity to recruit and deploy 16,000 new community health workers in Liberia.

ESSENTIAL JOB FUNCTIONS:

Accompany MOHSW officials

  • With members of the Health Systems Policy and Planning team, work alongside officials within the MOHSW’s Community Health Services Division to provide support for ongoing implementation and monitoring of the National Community Health Road Map.
  • Provide technical expertise to the MOHSW, with the support of other Last Mile Health teams, to design policies and implement national general Community Health Volunteer (gCHV) training programs, and ensure the national iCCM plan is underway.
  • With closely with the MOHSW to define a national gCHV curriculum, and design the national supervision package that includes tools and training for improved community health care delivery.
  • Work closely with officials within the MOHSW’s Community Health Services Division to launch a national incentives structure.

Support the MOHSW in coordinating with key stakeholders

  • Support the MOHSW in conducting coordination and planning meetings, developing implementation standards and supporting the MOHSW in establishing needed buy-in among partners.
  • Work with key stakeholders within the MOHSW to map and coordinate the work of NGO and technical partners within Liberia through active support and coordination of the Community Health Technical Working Group.
  • Participate in gCHV mapping activities and coordinate data collection within LMH and across partners in order to process and analyze the results.
  • Support the Incident Management System (IMS) and the national Ebola response as-needed.

Analysis and thought leadership

  • Provide the Health Systems Policy and Planning team with technical expertise and analysis to support advocacy efforts to direct funding towards the deployment of a national community health worker program and other key components of the Community Health Road Map.
  • Drawing on guidance from the Last Mile Health program and project implementation teams, conduct analysis of key outstanding key community health priorities, particularly those protecting gains made on MDGs and present findings to MOHSW colleagues via presentations, position papers, and other communications.

Uganda: Assistant Country Director - Systems

GOAL

Founded in 1977, GOAL works to promote the realization of the fundamental rights of the poor, the vulnerable, and those affected by humanitarian crisis. GOAL currently works in 15 countries; Ethiopia, Haiti, Honduras, India, Kenya, Malawi, Niger, Sierra Leone, Sudan, South Sudan, Syria, Uganda, Iraq, Philippines and Zimbabwe. GOAL’s head office is in Dublin, Ireland with support offices in London and New York.

General description of the programme

Operational in Uganda since the late 70’s, the current country programme focuses on two of GOAL’s three strategic sectors, namely: Health (including WASH and Health Accountability programming); and a market based approach to improving agricultural livelihoods. GOAL Uganda mainstreams HIV, gender, child protection and the resilience strategies across all programming. GOAL works on a district focused approach, and uses a mixture of direct implementation and partnerships with national and international civil society organizations, local government and private sector companies to give effect to our mission.

GOAL Uganda has a skilled team of approximately 120 personnel and manages an annual operating budget of approximately €3 million. GOAL is currently operational in the North, South, East and West of Uganda with five offices in Kampala, Bugiri, Abim, Kaabong and Agago. With a new livelihood programming planned we will open a new office in Lira in 2015.

The current GOAL Uganda country programme operational areas:

  1. The Bugiri / Namayingo District Programme, South Eastern Uganda, established in 2001, currently implements WASH and Health Accountability programme, both are implemented through national partners.
  2. The Agago District Programme, North Eastern Uganda, established in 2003 has transitioned from an emergency programme to a development programme. Implementation is through partners in WASH and directly in Health Accountability and both directly and through private sector partners in Livelihood.
  3. The Abim District Programme, North Eastern Uganda (Western Karamoja) was established in 2008. This development programme implements through partners in WASH and both directly and through private sector partners in Livelihood.
  4. The Kaabong District programme began in 2014 with a small WASH programme implemented through a national partner.

Description of the role:

The Assistant Country Director Systems (ACD-S) is a key member of the Senior Management Team at a country level, who will support and assist the CD in the overall management and leadership of the country programme. The ACD-S will assist the building and running of an organisation that can design, implement, monitor, evaluate and report on cost effective interventions of a high quality, consistent with relevant standards and norms. The ACD-S assists in the strategic planning and management of the country programme, working with programme and project teams and head office staff to define and implement a programme that gives meaning to GOAL’s mission, policies and procedures, within the Ugandan context.

The ACD-S will be expected to have a good understanding of the overall country programme, and be able to effectively deputise for the CD when s/he is on leave or absent, and to effectively represent the organisation and the country programme to external stakeholders.

The ACD-S will have responsibility for sections of the country programme including Human Resources and Administration, Logistics, Internal Audit, Communications and I.T. The ACD-S will have a focus on systems development, documentation and implementation, including working with HR on an effective in-house training programme and with Logistics and HR on audit systems for GOAL and donor policies. This will include the effective management of the GOAL asset register by facilitating the co-ordination of Logistics and Finance information.

The role of the ACD-S will be to coordinate and supervise the Logistics Coordinator, HRM, IT and compliance departments and to agree strategic priorities so as to ensure the efficient and effective implementation of GOAL’s programmes in Uganda.

Key tasks and responsibilities:

General Responsibilities:

· Develop a good understanding of the overall country programme;

· Deputise for the CD when CD is absent;

· Represent GOAL and the country programme, where relevant to external stakeholders;

· Agree with CD monthly and annual objectives, and provide CD with a monthly report on progress;

· Act as part of the Senior Management Team of the country programme, and chair meetings in absence of the CD;

· Perform other tasks, as agreed with the Country Director.

Human Resources and Administration:

Finance:

· Coordinate with the Country Finance Team to ensure that field core cost budget management tools are provided for review and management;

MAJOR DUTIES AND REPONSIBILITIES

A. Provide Guidance on Policy and Programme Development and Implementation

-Assist in co-ordination of the planning and delivery of integrated policy, programme and technical support and advice to Country Offices (COs) in the programming process;

-Technically support country office planning and implementation processes regarding health systems’ strengthening to increase access, uptake and monitoring of quality SRHR services and with a specific focus on HRH and health financing;

-Assist in policy dialogue and development within the region to further the advancement of the ICPD-PoA and health system strengthening;

B. Capacity development

-Identify capacity building needs of the country offices with regard to health system strengthening and follow-up through organization of capacity building activities with country office staff, national counterparts, and civil society partners; and

-Identify and support national and international experts and institutions in providing technical support to countries in the various areas of health system strengthening;

-Coordinate with other UN agencies, the provision of technical and programme assistance in the region and capacity development initiatives.

C. Monitoring and Oversight

-In coordination with the M&E adviser in the Regional Office (RO), and with regard to health system strengthening and SRHR, support the COs in the development and implementation of result****s****-based indicators and plans to monitor, evaluate and follow-up;

-Monitor and provide oversight to the development of key programme or management related documents from the assigned COs (e.g. Office Management Plan (OMP), Annual Work Plan (AWP), Country Office Annual Report (COAR), and evaluation reports, Annual reports of UNRCs, etc.) to keep abreast of country situation and progress, to identify areas for support by RO and to advise Regional management of issues, opportunities, and/or required interventions;

-Assist in the development of periodic progress reports on the regional programme, especially where health system strengthening is concerned.

D. Knowledge Management:

-Prepare briefings for the RO Deputy Director, Programme and Policy Advisors and COs on issues related to health systems’ strengthening at country and regional level;

-Support and assist the assigned COs to identify opportunities or lessons learned/best practices in the region with regard to efforts aimed at advancing SRH outcomes by strengthening health systems;

In response to the recommendation of the Global Survey on Violence against Children, commissioned by the Secretary General of the UN in 2006, UNICEF, along with the Centre for Disease Control Atlanta, is supporting the Government to carry out a comprehensive survey to understand the context, magnitude and prevalence of violence against children in Nigeria. Nigeria is one of the largest countries where the study is being undertaken and also the first West African Country.

Fundamental to preventing and responding to violence against children is a robust and adequately staffed and resourced child protection system. The current child protection system in Nigeria is typically characterized by fragmented services, limited coordination and unclear roles and responsibilities especially among government ministries. Child protection is significantly under resourced.

The Federal Government of Nigeria has recently embarked on a process to strengthen the social welfare and child protection systems – a programme of reform which UNICEF is supporting.

The purpose of this position is to:

  • Coordinate all current efforts within child protection and amongst traditional and non-traditional child protection actors, to ensure that these efforts collectively contribute to the strengthening of the child protection system at National and State level in order to effectively prevent and respond to violence, abuse, exploitation and neglect of children;
  • Support the Federal and State Government to launch the Violence Against Children Study and response plans; and
  • Coordinate and provide technical support for the implementation of the VAC response plan and the recommendations of the child protection systems mapping and assessment.

Short-term Expert in Health Information Management Systems (HMIS)

Field of activity

Within the framework of EUNIDA, a grouping of EU Member State implementing agencies, GIZ International Services is managing the EU Libya Health Systems Strengthening Programme.

Background

The Libyan health care delivery system reflects past systemic imbalances and the impact of the civil war. Weakened health services are largely deficient in their capacity to attend to the needs of the population. The increased prevalence of mental disorders and physical disabilities –a consequence of the conflict– further exacerbate the situation.

The overall objective of the EU-LHSS programme is to improve efficiency, effectiveness and quality of health service delivery in Libya. The purpose of this action is to strengthen strategic planning, financing as well as management and quality of health service delivery through targeted reforms and pilot interventions.

The EU-LHSS programme is articulated around three result areas:

  • Result One Strategic Planning, Health Financing and Capacity Strengthened: National health strategy, related policies and action plans developed; health financing options identified; provider payment mechanisms reviewed; and capacity of the Ministry of Health and Health Professionals Associations strengthened at all levels.

  • Result Two Health Service Delivery and Quality of Health Care: Health Service Package successfully rolled out in pilot areas; options for public private partnerships identified; quality of outcomes framework and accreditation standards developed and selected quality improvements reported.

  • Result Three Workforce Planning, Development and Management: Recruitment and retention targets in place; pre-service and in-service training needs assessed and partially addressed through targeted training and partnerships with training institutions; curricula upgraded and standardised; licensing standards and management tools developed and rolled out in pilot areas.

As part of the programme, model primary health care practices should be established in at least six PHC centres and six polyclinics. Selected interventions will be accelerated to help Libyan organisations and frontline workers make breakthrough improvements in quality of care and disseminate early results where they exist. In particular, the following priority areas have been selected for testing and implementation: health education and promotion, infection control, emergency departments management and triage, blood and blood products management, clinical protocols and guidelines, and laboratory-related quality improvements.

Accurate and reliable data is essential for policy making, health reform, regulation, education, health service and human resource management . Accurate and trustworthy information is simply the basis for all strategic decision making across the health care system both at national and local levels. A seamless process of health information management starts with timely data generation and collection, followed by structured compilation, synthesis and analysis.

In Libya, data collected is often not put to good use for analysis, monitoring or policymaking. There is no clear national HIS strategic plan and there is a severe shortage of trained IT staff that can develop local and national IT systems. The necessary legislations that should underpin any health care information management are lacking, particularly in as far as data protection and disclosure, and there are no national policies or guidelines on data collection and management. The data collected by the health care system is rarely systematically analysed to produce reports that can provide the basis for strategic decision-making. There are no national databases to collect information about the performance of health care professional or institutions to benchmark them at a national level.

In order to develop a modern nationwide Health Information System, a national HIS strategy and plan are urgently required. The plan should be costed and sequenced and a clear funding stream should be identified for the development of information technology resources and infrastructure. Open systems built-up over time, started as small projects have more chances for success than large scale, costly IT projects. Preference should be given to agreeing and managing systems locally, rather than imposing systems from above and the goal should be to continuously enhance systems to improve clinical decision-making and performance outcome.

IT staff should be recruited to work side by side with clinicians to develop and manage local and national heath IT systems, train clinical staff and develop their skills for the effective utilization of the system. Libya should also urgently adopt nationally agreed data collection and management standards and coding systems e.g. ICD10, procedure codes, unique patient identifiers and staff code identifiers. Legislation should be developed to govern data architecture, protection and disclosure.

Objectives of the mission

General objective of the mission

The Short-Term Expertise (STE) described in these Terms of Reference is complementary to the tasks of the long-term experts and responds to a request articulated by the Working Group (WG) on Health Service Delivery and the Ministry of Health.

The expertise mobilised will support the chair of the WG on Health Service Deliveryand the HIS Department of the Ministry of Health to:

  • Assess existing health management information systems, recommend and implement priority strengthening initiatives to support HIS requirements of health service delivery and improvements in quality of care at primary health care and hospital level.

  • Service HIS requirements for pilot-related monitoring, evaluation and operational research through pilot testing and scale-up of HIS in selected pilot sites.

Specific objectives of the mission

HIS Scoping Study


i) Assess existing Health Information Systems (HIS) in Libya including the accuracy and reliability of patient demographic and clinical data, emerging trends such as interface and interoperability risks.

ii) Assess existing needs for a customized HIS at the PHCs including data needs, access, acceptability, usability and ease of implementation.

iii) Analyse the scope for a unified and centralized system to be put in place and provide a menu of options for the operationalisation of such a plan including estimated costs, timeframe, knowledge, skills and resources required for a full roll out (turn-key).

iv) Respond to requests for specialist-level advice and assistance in the area of HIS policy, strategic planning, development and implementation, training, as well as in monitoring and evaluation

Design of a HIS for data capture in selected pilot sites


i) Assess user needs and expectation of system requirements, feasibility of implementation, and acceptability of adoption.

ii) Assist staff from the Ministry of Health and frontline workers in selected pilot sites to develop a monitoring and evaluation (M&E) plan, including indicator development, indicator definitions and collection of baseline information on service provision, quality of care and logistics (medicines and medical supplies).

iii) Design, develop and roll out a HIS that improves data capture, data flow, data use and storage related to health service delivery in Primary Health Care (PHC) pilots and selected quality improvements at PHC level. Data captured should include patient demographics, services provided, clinical management and clinical indicators, diagnosis of patients seen, utilisation rates, drug usage, staffing, administration and management.

iv) Provide training, tools and capacity development to streamline data collection, data management, data analysis, reporting and utilisation.

v) Apply HIS expertise to prepare evidence-based syntheses of baseline data for selected performance indicators in PHC pilots where quality improvement activities are being implemented.

vi) Oversee on-going monitoring and reporting against pilot indicators and results.

vii) Institute data quality assurance processes for continuous data quality monitoring.

viii) Provide training, tools and capacity development to upgrade the capacity of Ministry of Health staff, in particular staff from the Regional Office, to supervise staff responsible for routine data capture and data entry; and

ix) Assist the Ministry of Health and frontline workers with the preparation of data summaries and reports in user-friendly, graphic manner for routine reporting and broader dissemination;.

Management of the mission

Work language: The work language is English.

Responsibilities

In the absence of the Technical Assistance team and taking into account the nature of the mission (STE), the expert will report directly to the chair of the WG on Health Service Delivery and the HIS Department of the Ministry of Health.

The expert will also ensure close consultation with the Team Leader/Health Economist and chairs of other Working Groups, the Consultant (EUNIDA) and the Project Manager (the European Union).

Timetable

The duration of the mission is up to 72 working days and take place during 2014. The mission might be divided into 2-3 visits of 4-5 weeks duration each spread over a one year to allow time for training, gradual implementation and outcome assessments.

The tasks to be completed by the expert in total are as follows:


- Validate the outline of the report to be submitted including projected division of tasks by each of the three missions The outline will be reviewed and confirmed by the chair of the WG on Health Service Delivery, the chair of the WG on Quality of Care and the HIS Department of the Ministry of Health and address headings 1) – 2) and subheadings i) – xi) described under section 3.2. The outline will assist all parties clarifying deliverables and expectations.

- Discuss the findings of the situation analysis, the scope for a unified HIS system and the options available.

- Conduct a workshop/meeting to validate the M&E plan, the proposal for roll out and the plan of action in selected pilot sites.

- Submit an intermediate report that should be circulated for comments amongst members of the WG on Health Service Delivery and HIS Department of the Ministry of Health, and chairs of other Working Groups (if deemed relevant).

- Provide training and tools to Ministry of Health staff and frontline workers in pilot sites to streamline data collection, data management, data analysis and utilisation and includes working with the Champion of medical record improvement through the Quality of Care Working group.

- Launch the HIS in selected pilot sites and organise a one-day workshop to present baseline data collected.

- Provide quality assurance processes and tools including guidelines for routine data capture and data entry and supervision.

- At least three days prior to the end of each mission the expert shall deliver a report of the work accomplished during that particular mission, which should have incorporated feedback from the chair of the WG on Health Service Delivery, the chair of the WG on Quality of Care and the HIS Department of the Ministry of Health and the plans and date for the next mission.

- A minimum of 10 years consultancy experience in facilitation of health information system strengthening, including central level coordination and systems integration, and sub-national strengthening for quality collection, analysis, dissemination and use of health information.

- Proven experience in designing, developing, implementing and maintaining information systems in order to monitor, support and evaluate large-scale programme activities.

- Proficiency in training and capacity building to enable managers and institutions to improve information systems and utilise data for decision making.

- Solid understanding of the role of HIS in supporting planning, management and evaluation.

- Experience and skills in identifying strategic information needs, collecting, analysing and synthesising data and presenting data in a succinct and user-friendly manner.

- Ability to understand and assimilate new Health Management Information Systems approaches, as well as health programme design, management and implementation approaches.

- Strong communications skills to ensure effective working relationships with government institutions, national and international working partners.

- Proven writing and editing skills, with a strong command of English and ability to convey complex ideas in a clear and direct style.

- Previous experience working with District Health Information Systems (DHIS) highly desirable.

- Working knowledge of Arabic desirable.

Reports

The report should specifically address headings and subheadings outlined under section.
The report shall also summarise the main activities implemented, potential constraints encountered, the measures taken or to be taken (by the expert, the EU LHSS programme or the Ministry of Health) to ensure the achievement of the objectives and the expected results. Further, the report shall deliver an analysis of potential gaps with regards to the expected results as well as recommendations for the roll out of the basic health care package.

Delivery and approval of reports

A draft report will be delivered electronically to the Technical Assistance Team. All documents delivered by the expert shall receive prior approval by the chair of the WG on Health Service Delivery, the chair of the WG on Quality of Care and the HIS Department of the Ministry of Health. It will further be approved by the Project Manager (the European Union) and at the Consultant (EUNIDA/GIZ).Assignment period: up to 72 working days completed in two to three visits over a one year period.The intended commencement date is January 2014
